The registered nurse provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process. Duties will include, but are not limited to the following:
Conduct screening, assessment, treatment, monitoring, counseling and supportive services.
Educate patients, family members or other supportive individuals about Suboxone therapy as well as risks, benefits, potential side effects, interactions, program requirements, consents and treatment contracts.
Involve patients in the development of the treatment plan and working with the patient and interdisciplinary team to individualize the plan.
Enhance treatment readiness, supports treatment completion, ensures safety and promotes sustained recovery outcomes.
Improve access, identify and provides information about community resources and assists patients in accessing other treatment options as needed.
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge and supporting factors that impact customer satisfaction.
Work Schedule: Monday-Friday 8:00 a.m. to 4:30 p.m.
